What is High Tibial Osteotomy?

High tibial osteotomy (HTO) is a surgical procedure performed to realign the bones in the knee joint, specifically the tibia (shinbone), in order to relieve pain and improve function in patients with certain types of knee conditions, particularly those with early-stage osteoarthritis or certain knee deformities. The procedure is aimed at shifting weight-bearing forces away from the damaged or affected part of the knee joint to healthier areas, thereby reducing pain and slowing down the progression of arthritis.
During a high tibial osteotomy, the surgeon makes a carefully planned cut in the upper part of the tibia. This cut allows for the bone to be realigned either by opening a wedge on one side of the bone (called a “closing wedge” osteotomy) or by removing a wedge from the other side (called an “opening wedge” osteotomy). The choice between these techniques depends on the specific needs of the patient and the surgeon’s preference. Once the bone is realigned to the desired position, it is stabilized using metal plates, screws, or other fixation devices.
The goal of high tibial osteotomy is to redistribute weight more evenly across the knee joint, thereby reducing pressure on the damaged area and delaying or preventing the need for more invasive procedures such as knee replacement surgery. This procedure is typically recommended for younger, active patients with early-stage knee arthritis or specific types of knee deformities that are not amenable to conservative treatments.
Recovery from high tibial osteotomy involves a period of immobilization followed by gradual rehabilitation, including physical therapy, to regain strength, stability, and range of motion in the knee joint. It’s essential for patients to follow their surgeon’s post-operative instructions carefully to optimize the outcomes of the procedure. While high tibial osteotomy can provide significant relief for some patients, it may not be suitable for everyone, and the decision to undergo this surgery should be made in consultation with a qualified orthopedic surgeon.
